Nurse Practitioner – Upto 0.5 FTE – MOSH 

  • Location: 2131 Gottingen Street
  • Hourly Pay: $52.41/hr. – 61.27/hr.

Reporting to the Mobile Outreach Street Health (MOSH) or NECHC Primary Care Manager, the Nurse Practitioner provides health care service as a member of the collaborative MOSH team that includes physicians, nurse practitioners, nurses, occupational therapists, health case managers and others to a designated client population group requiring focused health and illness care. The nurse practitioner is licensed in a nurse practitioner class pursuant to the regulations of the Nova Scotia College of Nurses. The practice of a nurse practitioner includes the provision of specialized health services, which include harm reduction, health promotion, illness, and injury prevention, coordination and management of acute and emergent health problems, and education and advocacy relevant to the patient population. Location of work would primarily include a clinical setting at the North End Community Health Centre (NECHC), and other NECHC and community sites which may include homes. Work with MOSH might also include outreach with MOSH Nurses from a mobile clinic or partner location in the community.
 
The following are the main objectives of the MOSH program:
  • To increase access to effective and equitable primary health care to community members across the lifespan who are homeless, street-involved, and insecurely housed.
  • To improve the health status of marginalized and underserved populations.
  • To create a collaborative environment in which information, knowledge, and resources are shared among community service, health care, academic, and government organizations. 

Qualifications:

  • Master of Nursing – Advanced Nursing Practice stream.
  • Eligible for licensure as an RN and as a Nurse Practitioner with the College of Registered Nurses of Nova Scotia.
  • Valid Driver’s License

 Social Worker (BSW) – Brunswick Street – DESIGNATED**

The Social Worker is a professional within the field of social work who focuses on addressing the psychosocial and emotional aspects of community development and engagement for clients residing at the Supported Housing location – Brunswick Street. Their role is to coordinate and facilitate therapeutic and leisure activities to improve clients’ physical, mental, emotional, cultural and social well-being; as well as support tenants to build community connections within the Overlook and outside of it. This Social Work position will assess clients’ needs and collaborate with the interdisciplinary team at the Supported Housing location – Brunswick Street – to ensure that clients have opportunities for engagement that considers both their individual and collective mental health needs and their psychosocial well-being. 
 
The Social Worker will provide programming primarily focused to build community connection and relationships within the building through dedicated programming (80%) and also facilitate connections for the Brunswick Street clients to the larger community outside of the building through land-based activities, recreational field trips, opportunities to attend events and other community spaces (20%). 
 
The Community Development Social Worker works from an equity lens and understands the needs of community members experiencing homelessness, poverty, substance use, and marginalization related to trauma, race, gender identity, disability and criminalization. They have skills in system navigation, strength-based interventions, and the ability to interpret and use provincial and federal policy to support clients on issues of poverty, housing maintenance, healthcare access, harm reduction, food insecurity, Income Assistance, pharmacare and other benefits.
Qualifications:
  • Bachelor of Social Work (BSW) from an accredited University.
  • 2-5 years’ experience preferred
  • Licensed and in good standing with Nova Scotia College of Social Workers (NSCSW).
  • Understanding of the impacts of the social determinants of health.
  • Experience and skill in group facilitation and programming coordination.
  • Practice rooted in person-centered, anti-Black racism, anti-oppression, harm-reduction, trauma informed care and decolonization.
  • Uphold provincial and federal ethical standards of practice (NSCSW and CASW).
  • Knowledge and experience of working with people who have complex needs and who are experiencing substance use, homelessness and/or poverty.

 Assistant Manager, Brunswick Street – DESIGNATED** – (Non-Unionised)

The Assistant Manager at Brunswick Street will provide strategic leadership and operational oversight to the housing team at this supported housing initiative, which is operated by the North End Community Health Centre and owned by the (SVDP). The multidisciplinary team provides wrap-around services to seniors who have experienced homelessness with health concerns to age in place.
 
Reporting directly to the Manager of Brunswick Street, the Assistant Manager will be responsible for supervising, mentoring, and supporting supported housing staff and peer support workers in the delivery of housing support to the 38 tenants within a multi-disciplinary, collaborative team. The Brunswick Street team is committed to fostering a diverse, inclusive environment and actively promotes a collaborative, community-centered approach to care.
 
Qualifications:
  • 5 years of community-based health experience, or equivalent setting
  • 2+ years of experience leading and supervising staff, including mentoring, orientation and training, job performance feedback and motivational job performance planning
  • Strong experience in de-escalation and conflict resolution skills
  • Experience in developing and coordinating client programs and program materials
  • Experience with project/program budget and/or funding opportunities and grant oversight
  • Community and street-based outreach experience
  • Strong computer literacy, including familiarity with common software programs.

The NECHC’s Property Management group is responsible for the maintenance and property management of the housing portfolio of the North End Community Health Centre. Reporting to the Property Manager, the Property Assistant is responsible for supporting our housing facilities, providing maintenance and other duties as required. The team values diversity and proactively seeks ways to incorporate a collaborative community approach to all that they do.
 
The Property Management Assistant will provide day-to-day, on-site support to our Brunswick Street Supported Housing project, a 38 unit building for individuals who have experienced homelessness who are over 50. This project is a partnership with SVDP who own the building and will provide tenants with complex health care needs the opportunity to age in place with supports.
 
Qualifications:
  • 1-2 years’ experience working in a community setting with community members.
  • Experience being a handy person is a requirement
  • Strong skills related to: adaptability, time management, problem solving, judgement,communication, interpersonal relations, and healthy boundaries.
  • Compassionate, non-judgmental approach to supporting clients whose needs and goals may bedifferent than your own, creative thinking, ability to navigate a crisis situation, de-escalate, and/or pull in more support.
  • Valid driver’s license required, with a minimum of 2 years driving experience and clean driverabstract.
  • Access to a reliable vehicle is required.

Dietetic Aide

As a member of the primary care Health Team, the Dietetic Aide is responsible for the support in developing, implementing, and evaluating community-based nutrition programs. Qualifications:
  • Bachelor’s degree in nutrition

Community Nutritionist, RD

The Community Nutritionist reports to the Manager of Allied Health – Primary Care as a member of the Community and Mental Health team. The Community Nutritionist is responsible for developing, implementing, and evaluating the clinical nutrition program while supporting and overseeing the community- based component.
Clinical programming is provided in a primary care setting with an interdisciplinary health team of physicians, nurse practitioners, nurses, dentistry, and other allied health professionals. The Community Nutritionist provides individual nutrition counselling services for clients of NECHC programming, including but not limited to chronic disease prevention and management, special diet letters, and resource navigation. The clinical component may also include the development and facilitation of group programming based on the needs of the community.
The Community Nutritionist will also play an active role in community programming. This includes oversight and collaboration with a Dietetic Aide in the management and delivery of a monthly produce program, a local food network, and various short-term grant-dependent food programs.
The Community Nutritionist works Monday to Friday, 9am – 5pm.
 
Qualifications:
  • Bachelor of Science in Applied Human Nutrition with successful completion of Dietetic Internship Program required.
  • Licensed and in good standing with Nova Scotia College of Dietitians and Nutritionists, required
  • 1-3 years working experience as a Registered Dietitian, healthcare setting preferred
  • Strong knowledge of North End Halifax
  • Understanding (or willing to learn) Afrocentric and/or Indigenous approaches to nutrition and food, required.
  • Experience working with communities with historical and current trauma and oppression.
